What to do
Step 1: Read the remark code
CO-197 / “auth required” is different from “does not meet criteria.” Fix process errors with retro auth; fix clinical denials with records.
Step 2: Check if the setting waived PA
Emergency and many inpatient services have different PA rules. Attach the ER record if care could not wait.
Step 3: Use the vendor and the plan
If eviCore or Carelon denied PA, appeal there and keep the health plan appeal rights on the EOB.
Step 4: Submit a complete packet
Order, clinical notes, codes, and the PA ID. Incomplete packets are denied again.
Step 5: Ask for expedited review when delay risks harm
Many plans decide urgent PA appeals on a short clock (often 72 hours). Request it explicitly.

- EOB remark codes
- Clinician order and notes
- Proof of submission / portal screenshot if PA was filed
PA appeal windows are on the determination notice and may be shorter than a post-service claim appeal. Treat the notice date as day one.

- Starting a brand-new PA instead of appealing the denied one (you lose the record)

FAQs
Can I get PA after the service?
Sometimes, especially for emergencies or when the plan’s rules allow retrospective review. Attach clinical urgency. Elective services done without PA are harder to salvage.
How long does PA take?
Non-urgent PA is often days to two weeks; urgent requests are faster. Your plan’s evidence of coverage states the clock. A delay is not itself an approval.
Is a peer-to-peer the same as an appeal?
Peer-to-peer is a clinical discussion that may overturn a PA. Still file a written appeal before the deadline so you do not lose the formal right if the call fails.
What if the office forgot to submit PA?
Ask the office to request retro auth and write what happened. Some plans still deny. You can appeal as the member; the office should cooperate with records.
Does Medicaid use PA too?
Yes. Follow the Notice of Action. State fair hearing rights may apply in addition to the plan appeal.
Should I wait to have the surgery?
If the service is elective and PA is denied, wait for approval or a successful appeal unless the clinician documents urgency. That is a clinical decision, not an AppealFlow recommendation.
